Job Summary:

The Advanced Practice Nurse (APN), in collaboration with the Physician Advisory Committee, will consult with patients, and coordinate palliative care services among a team of professionals including care managers, social workers, and pastoral care service. 



Education Requirements:

  • Advanced Practice Nurse licensed in IL
  • Work experience in acute care, oncology, ambulatory care, pain management, hospice/home health and/or Palliative Care (preferred)
  • Knowledge of end of life care, including advanced care planning, pain and symptom control of the terminally ill, and bereavement care issues desired.
  • Board certification as an Advanced Practice Nurse required; ACHPN credentialed or desire to work toward certification
  • Prescriptive Authority licensed in the state and in accordance with signed  collaborative agreement, if applicable
  • CPR certification
  • Minimum of two years of RN/APN experience preferred 
  • Excellent interpersonal and communication skills


Experience & Special Skills:

  • Must be skilled in diagnostic and treatment methods and protocols common to patients in the palliative care service.
  • Possesses advanced nursing assessment skills.
  • Experience in providing holistic care
